Enrollment Forms

Swift River enrollment forms (pdf) must be signed before the day of enrollment. We ask that you bring these documents, along with the items listed below upon your arrival on Day of Enrollment. If you have any questions, please contact the Admissions Department at (800) 258-1770 x209, and we will be happy to clarify for you.

Please submit the following items with your enrollment forms:

• Copy of the birth certificate
• Copies of health insurance cards — (front & back)
• Immunization/Vaccination records from your child's Physician
• Phone Card
• Copy of your custody agreement as identified in the divorce decree
• Wilderness Discharge Summary from the wilderness program attended
